I wear a medium in Hanes Ts, but a large here for a "fitted" fit and XL for a looser fit.

I'd suggest a large. I'd say men's sizing runs about right/a little small. It'll show the biggest difference after the wash/dryer.
My brother wears a large here and he wears a large in everything, but!!! He also likes that woot shirts are much more fitted than his shirts from elsewhere, so there's definitely at least a half-size too small thing going on. So it all depends on how the guy for whom you are purchasing likes his shirts to fit. I'd say XL for loose and baggy, L for slightly fitted.

It's Gozer, actually, as the Traveler, while Stay-Puft on the right represents Gozer as the Destructor. The dog itself would be Zuul (Dana) given the word below as opposed to Vinz Clortho (Louis).
