The episode that comes to my mind when I see this is Leisure Hive. K-9 fries in the water chasing a beach ball at the very beginning of the episode, while the 4th Doctor is snoozing in a chair.
Yes, the phone door is incorrectly colored, but it's not a huge deal. I doubt more than 20% of Who fans notice that anyway.
