I can see the how (he's got the sword in his left arm upside-down and the arm held across his chest over the pistol), the why is what escapes me. If he had it under the pistol, steadying it, I could understand; but that pose just seems like it would hinder aim, rather than help it.
Ahh. I think I got it now. He's close enough to his opponent that aim is irrelevant. He drew his pistol right after the bloody slice and surprised his dazed sword fighting opponent with a chest shot.
